From routine clinic visits and prescriptions to understanding a new diagnosis, we have a variety of benefits to help support your mental and physical health.

Medical insurance

If you’re an employee who works at least 20 hours per week, you’ll choose between two health plans. Our plans provide comprehensive medical coverage, pharmacy benefits and 100% coverage for age-appropriate, network preventive care. We also offer two additional HMO plans in select geographic areas. Eligible employees also may cover their spouses/domestic partners and children.

To comply with the Transparency in Coverage Rule, UnitedHealthcare creates and publishes machine-readable files with pricing data on behalf of U.S. Bank. 

Dental and vision plans

Our dental options encourage proactive preventive care and help offset the cost of major services. Our vision plan provides coverage for routine eye exams, glasses and contact lenses, plus discounts on hearing aids and exams. Both dental and vision plans are available to employees who work at least 20 hours per week and their eligible spouses/domestic partners and children.

Well-being programs

In addition to 100% coverage for network preventive care through our medical plans, we also offer a variety of well-being programs and resources to help you reach your specific health goals. You can use them to make healthier decisions about nutrition and weight, prepare for a healthy pregnancy and birth, get a second opinion about a medical condition, quit smoking and more​​​​​​​.

Healthcare Flexible Spending Account

Available to employees who work at least 20 hours per week, a healthcare flexible spending account (FSA) allows you to pay for eligible health care expenses for you and your dependents using before-tax dollars. You contribute a portion of your elected amount each pay period and the entire annual contribution amount is available to you right away.

Health Savings Account

If you enroll in a U.S. Bank high deductible medical plan, you may be eligible to contribute to a Health Savings Account (HSA) to save and pay for deductibles, coinsurance and qualified health care expenses.  
 
With this account you contribute before-tax dollars, so you don’t pay taxes on the money you use to pay for eligible expenses, and your taxable income is lowered. U.S. Bank also contributes to the HSAs of eligible employees, which helps you save even more. Plus, your account is yours, even if you leave U.S. Bank.
